God has blessed us with vans that can be used by ministries within the church. To be good stewards of what God has given us and to make sure that everything is ready to go for your ministry’s trip, we ask that you please fill out our vehicle request form. 

We are very thankful for the opportunity to work with your student!  In order for us to go on fun adventures together, we need a Parental Consent Form on file for each student who will be participating with us at off site events.  Please print the form out and drop it off at the office after filling it out!  

To request reimbursement for an expense, fully complete the "Reimbursement/Check Request Form" and submit to the church financial secretary. 

In order for us to allow drivers to use our vans, we must first have one of these forms filled out for our insurance.  Please print the form out and drop it off at the office after completing!  

We are very thankful for the facilities that God has blessed us with here at First Baptist. Our goal is to honor God by taking care of our facilities to the best of our ability.
